Insurance FAQ

It is important to understand that insurance is a benefit and your coverage should not guide your dental treatment. We will recommend the best treatment for your dental needs, and give you an estimate of your insurance coverage before any treatment is started. Insurances can be confusing but we will be happy check all of your benefits and file your claims. Simply bring us your insurance card or provide all of your information when you schedule your appointment. 

Fee Schedule:

We are contracted with most insurance companies and have a set negotiated fee that your insurance allows us to charge you for each dental procedure. Some insurance plans cover part of these fees and you will be responsible for the remaining balance the day of your appointment. If you have a discount plan, you are responsible for the full fee the day of your appointment. We will verify your insurance prior to your appointment and let you know what they cover.

Financing options available through Care Credit and Lending Club.

Copayment: 

A copayment is a monetary charge that your health insurance plan may require you to pay in order to receive a specific service. For example, your health insurance plan may require a $25 copayment for an office visit.

Deductible:

The insurance deductible is the amount of money you will pay in an insurance claim before the insurance coverage kicks in and the company starts paying for your dental needs. For example, if your deductible is $50, you must pay that amount before your insurance covers any dental work. Once that amount is met for the year, you will no longer have to pay it.

Yearly Maximum:

This is the amount of benefits your insurance will cover in a calendar year. If you do not use your benefits in a calendar year, you will lose them and they will renew the next year. If your insurance provider does not pay for their estimated portion, we will bill you for the remaining balance. However, we will submit all your records and x-rays necessary to optimize your benefits when we file a claim.

VIP Membership $250/ year

Includes comprehensive and periodic exams (D0150, D0120), x-rays (D0210, D0274, D0330), 2 cleanings (D1110), and up to 20% off all services.
Cannot be used with insurance plans or other discounts. Only for patients with no gingival or periodontal disease.

Periodontal VIP Membership $325/ year

Includes comprehensive and periodic exams (D0150, D0120), x-rays (D0210, D0274, D0330), 3 cleanings (D1110, D4910), and up to 20% off all services.
Cannot be used with insurance plans or other discounts. Scaling and Root Planing not included. Only for patients with stable periodontal disease.
